Alert: Community HealthChoices Southeast Plan Selection Deadline Approaching

 Community HealthChoices (CHC) is Pennsylvania's Managed Long-Term Services and Supports program that will be launching in Bucks, Chester, Delaware, Montgomery, and Philadelphia counties at the beginning of the New Year. 

 CHC is for individuals 21 or older who qualify for Medicaid and Medicare and require long-term services and supports. The program was developed to improve access and coordination of these medical services.   

In a news release announcing the Southeast selection deadline, Teresa Miller, Department of Human Services Secretary said, "Our goal is always to enhance the quality, efficiency, and effectiveness  of the supports and services available to Pennsylvanians."

With the launch of CHC in Southeast Pennsylvania, eligible residents were notified and instructed to select their chosen Managed Care Organization (MCO). These eligible residents now have until December 21 to make their final decisions on plan selection changes. The participating MCO providers are Keystone First Community HealthChoices, PA Health and Wellness, and UPMC Community HealthChoices.

Community HealthChoices' launch in Southeast Pennsylvania will include around 131,000 individuals and is expected to include 420,000 individuals once it is fully implemented across the state. 

"The Wolf Administration encourages residents of Southeastern Pennsylvania who are enrolling in Community HealthChoices to review their MCO selection and make any changes before December 21," said Secretary of Aging Teresa Osborne.  She goes on to state, "It's important to us that each participant is engaged in the process and enrolls in the plan that best meets their needs."
